What You’ll Do

• Provide direct patient care, including administering medications, treatments, and managing conditions

• Develop and implement individualized care plans in collaboration with the care team

• Conduct thorough patient assessments and respond to changing needs

• Communicate effectively with patients, families, and team members to ensure seamless care

• Maintain accurate, timely documentation in compliance with regulatory standards

• Participate in quality improvement initiatives and support best practices

• Collaborate on education efforts that empower patients and promote better outcomes

What You Bring

• Active Registered Nurse (RN) license in the state of Oregon

• Strong communication and interpersonal skills

• Ability to thrive in a fast-paced, team-oriented environment

• Solid clinical knowledge in geriatric care, rehabilitation, and wound management

• A compassionate, patient-centered approach to care
