Responsibilities

• Lead the Social Services department in providing high-quality care and services to our residents, ensuring that their physical, emotional, and social needs are met

• Develop, implement, and evaluate individualized care plans in collaboration with the interdisciplinary team

• Coordinate and facilitate social services activities, programs, and events that promote resident engagement, well-being, and quality of life

• Build and maintain strong relationships with residents, family members, and healthcare professionals to ensure effective communication and collaboration

• Identify and address resident concerns, complaints, and issues in a timely and professional manner

• Monitor and report on resident satisfaction, quality of life, and social services outcomes to inform departmental improvements and strategic planning

• Collaborate with other departments, including admissions, marketing, and quality improvement, to ensure a seamless and comprehensive care experience for residents
