About the Role

As a Licensed Practical Nurse (FT evenings) at The Pearl of Elgin, you will work in a dynamic and supportive environment, collaborating with our interdisciplinary team to provide high-quality care to our patients. Your responsibilities will include:

• Providing direct patient care, including administering medications, observing and reporting changes in patient conditions, and performing routine tasks as assigned
• Developing and implementing individualized care plans in conjunction with the healthcare team
• Participating in multidisciplinary care conferences and staff meetings to ensure seamless coordination of care
• Maintaining accurate and detailed records of patient interactions and care provided
• Collaborating with healthcare professionals to ensure patient safety and satisfaction
• Contributing to a positive and respectful work environment by demonstrating a commitment to teamwork and excellent communication skills

Requirements

• Current LPN/LVN licensure in the State of Illinois
• Strong understanding of patient-centered care principles and practices
• Ability to work effectively in a fast-paced environment with multiple priorities and demands
• Excellent communication, interpersonal, and organizational skills
• Compassionate and empathetic approach to patient care
