About the Role:

As an LPN at Lake Pointe, you will play a vital role in delivering high-quality care to our short-term residents. Your responsibilities will include:

• Providing direct patient care under the supervision of licensed nursing staff, including administering medications, collecting vital signs, and performing treatments

• Developing and implementing individualized care plans in collaboration with the interdisciplinary team

• Maintaining accurate and detailed records of patient care and progress

• Assisting with daily living activities, such as bathing, dressing, and grooming

• Participating in shift reports and communicating effectively with other members of the healthcare team

Requirements:

• Current LPN license in the state of Ohio

• High school diploma or equivalent required; associate's degree in nursing or related field preferred

• Minimum of one year of experience as an LPN; experience in rehabilitation or long-term care setting a plus
