Responsibilities:

• Provide emotional support and guidance to residents and their families, addressing their mental health and psychosocial needs

• Conduct psychosocial assessments and develop individualized care plans to meet residents' social and emotional needs

• Facilitate communication between residents, families, and healthcare teams to ensure comprehensive care

• Develop and implement interventions to address residents' mental health and cognitive issues

• Collaborate with interdisciplinary teams to ensure resident-centered care and effective discharge planning

• Maintain accurate records and documentation of social work services and interventions

Requirements:

• ASSOCIATES degree in Social Work or related field

• 1 year of experience in social work

• Licensure as a Social Worker in the state of Vermont

• Strong communication and interpersonal skills

• Ability to prioritize and manage multiple tasks and responsibilities

• Compassion, empathy, and a commitment to delivering exceptional care
