We’re hiring a full-time Director of Social Services to oversee our Social Work Department for long-term and acute care patients. Help residents thrive, guide families, and lead a team dedicated to exceptional care.

What You’ll Do:

• Lead social services for residents and families.

• Develop social histories, assessments, and care plans.

• Participate in interdisciplinary care meetings.

• Support discharge planning and placement.

• Ensure compliance with state and federal regulations.

What We’re Looking For:

• Bachelor’s in Social Work or related field (Master’s preferred).

• Strong interpersonal & leadership skills.

• Experience in long-term care or healthcare social services.

• Knowledge of state and federal social services guidelines.
