About the Role:

We are seeking a highly motivated and experienced Registered Nurse Clinical Manager to join our team. As a Clinical Manager, you will be responsible for coordinating and overseeing all direct care patient services provided by our clinical personnel. You will conduct patient assessments, develop and implement care plans, and ensure that our patients receive the highest level of care in a compassionate and respectful manner.

Responsibilities:

• Conduct patient assessments and reassessments, updating care plans and interpreting patient needs

• Ensure patients meet admission criteria and make decisions to admit patients to service

• Assign clinicians to cases as needed and assist in establishing immediate and long-term therapeutic goals

• Coordinate communication between team members, attending physicians, and caregivers

• Supervise clinical employees, carry out supervisory responsibilities in accordance with company policies and procedures

• Participate in interviewing, hiring, training, and development of direct care clinicians

• Review case managers and clinical staff documentation for appropriateness of care, delivery, and documentation requirements

Required Experience/Skills:

• Graduate of an accredited School of Nursing

• Current state license as a Registered Nurse

• Proof of current CPR

• Valid driver's license, auto insurance, and reliable transportation

• Medicare Home Health Experience REQUIRED

• Two years as a Registered Nurse with at least one year of management experience in a home care, hospice, or equivalent environment
